When to See an Orthopedic Specialist for Morning Back Stiffness

How many times have you woken up, rolled over, and realized your back is stiff and achy again? For many Fresno area residents, a stiff back is quickly written off as a sign of aging or your busy lifestyle.

You spent so much time in your car on the morning and evening commute after sitting behind a desk all day. You spent hours bent over weeding your garden beds. Or, you spent all weekend hiking in the foothills and sleeping on the ground in a tent. It’s easy to blow off the pain as part of your activities.

However, there is a big difference between occasional morning stiffness that fades after your shower and a chronic musculoskeletal issue that a doctor needs to diagnose and treat. Knowing when to go straight to an orthopedic specialist is critical if you want to prevent chronic pain and joint damage.

Understanding Morning Back Stiffness

Before you know if it’s time to seek a medical opinion, you should understand the two categories of morning stiffness and back pain: inflammatory and mechanical.

Inflammatory Stiffness

When you’re sleeping, inflammatory chemicals have the chance to take up space in the joints. It happens if you have an inflammatory or systemic condition.

While movement often eases back stiffness caused by mechanical issues, inflammatory stiffness isn’t easily reduced. The pain continues for upwards of an hour or longer. Medical treatment of the underlying condition is needed in order to ease at least some of the stiffness and pain.

Mechanical Stiffness

The mechanics of your back require discs, joints, ligaments, and muscles to work in sync. When you’re sleeping, nothing is moving. Synovial fluid lubricates your joints, but it requires movement to circulate that fluid and keep it thin and watery. As you’re stationary while you sleep, the fluid thickens.

Plus, the muscles aren’t moving, and your pulse slows while you sleep. Blood isn’t moving around as much, so muscles and ligaments tighten up.

Once you’re up and moving around, the muscles and ligaments loosen. Your synovial fluid thins, so the joints receive the lubrication needed to help ease the stiffness.

The best way to determine whether your stiffness is inflammatory or medical is to see an orthopedic specialist.

Common Spinal Conditions an Orthopedist Diagnoses and Treats

Orthopedic specialists study the entire musculoskeletal system. Pairing physical examinations and advanced imaging, they can determine if your morning back stiffness is caused by these spinal conditions or something else.

  • Bulging Discs: Weakening spinal cartilage allows a disc’s fluid to push outside of its usual boundaries, which puts pressure on the nerves. Swelling worsens after several hours without moving around, leading to stiffness.
  • Degenerative Disc Disease (DDD): Spinal discs absorb shock in the vertebrae. When the discs lose fluid, they become thinner. This can lead to nerve compression when lying flat, creating stiffness in the spinal column.
  • Herniated Discs: Similar to bulging discs, a herniated disc occurs when a small crack forms in the spinal cartilage. Pain is more common in a herniated disc.
  • Osteoarthritis: The slow deterioration of the protective cartilage on the joints linking your vertebrae creates arthritis. As the cartilage wears away, bones in the joint rub together and create inflammation and morning stiffness.
  • Spinal Stenosis: Sometimes, the spaces in your spine become narrow, pressing on the nerves and spinal cord. This triggers stiffness and, sometimes, leg and lower back pain.

What Else Could Be Causing Your Back Pain?

If it’s not a problem with the joints in your spine, what else causes stiffness?

Dehydration

Water is essential, and your muscles may cramp up if you’re not drinking enough. In Fresno’s summer heat, it’s important to drink enough water. Excessive sweating depletes your tissues’ water stores faster than you might realize.

Aim for around 3 liters a day if you’re a male, and 2 liters if you’re female. Increase that if you engage in strenuous activities. A good goal to follow is to have pale yellow urine. If it’s dark yellow, you’re not drinking enough. Premium Sports & Orthopedics offers IV drip therapy if your fluid levels need a boost.

Fresno Lifestyles

Fresno’s climate and location often get people out of their homes and into the mountains and hillsides. Poor shoes while hiking may cause strain on your ankles, knees, and hips, so your body has to overcompensate by using other muscles. That overcompensation impacts the muscles in your back.

Sitting Too Long

Long commutes down the 41 or 99 corridors or long deliveries from agricultural or industrial facilities can impact your spine. Sitting on a hard car seat for hours on end puts pressure on your lumbar area.

Moving from a cramped, hard seat with poor support to a soft bed negatively impacts your back. You’ll wake up with a stiff back.

Weather Extremes

While barometric pressure and temperature changes don’t cause spinal diseases, they can heighten your discomfort. Cold, rainy winter weather can make your muscles contract, making your back feel even stiffer than it is.

Knowing When It’s Time to Move Past Home Remedies

If your morning routine always starts with stiffness and back pain, you need to determine when it’s time to see an orthopedic specialist. We recommend it when:

  • Pain and stiffness have been around for more than six weeks.
  • Stiffness lasts longer than 45 minutes.
  • Symptoms worsen when you’re not active.
  • You experience neurological symptoms like numbness and tingling.
  • Your legs, knees, or feet become too weak to use.

What to Expect at Your First Appointment With an Orthopedic Specialist

You’ve decided to see an orthopedic specialist. The initial visit is making you nervous. What should you be prepared for?

  • Patient History: Much of this initial appointment will be spent getting to know you. Your doctor wants to know your medical history and any details you can share about your family’s health history. If you’ve had images taken, your doctor will go over them. Be prepared to answer questions about the stiffness you experience and activities leading up to it.
  • Physical Evaluation: The doctor will look at your spinal alignment by having you bend forward, lean back, and move side to side.
  • If you decide to take control of your symptoms and see a specialist, your first visit will be comprehensive. The goal is to establish an accurate structural roadmap of your back.
  • Advanced Imaging: If you haven’t had an MRI or X-rays of your back completed, those will likely be scheduled or even completed that day.
  • Assessment: Once the doctor knows what’s causing your back stiffness, a treatment plan is established. It might mean non-surgical options like physical therapy or treatments for an underlying condition.
